Evolysse Form and Smooth: What These Fillers Actually Are

Evolysse Form and Evolysse Smooth are FDA-approved injectable hyaluronic-acid dermal fillers, not skincare. What they are, the approved indication, and how Revive Beauty Bar in Salinas, CA selects filler.

A correction, first. An earlier version of this article described Evolysse as something you apply to your skin at home. That was factually wrong and has been rewritten. Evolysse Form and Evolysse Smooth are injectable hyaluronic-acid dermal fillers, administered by a qualified provider during an in-office appointment. They are not a serum, not a cream, and not something you apply yourself.

What Evolysse Is

Evolysse Form and Evolysse Smooth are FDA-approved injectable hyaluronic-acid dermal fillers from Evolus. They are supplied as sterile gels and are injected into the dermis or subdermis by a trained injector during an in-office appointment.

Hyaluronic acid is a sugar molecule your body already produces. In a dermal filler it is cross-linked into a gel so that it holds its shape in tissue rather than dissolving immediately. Different fillers are cross-linked differently, which is why they vary in firmness, in how much water they attract, and in how they behave in a mobile area such as the lips compared with a structural one such as the chin.

The Approved Indication

Evolysse Form and Evolysse Smooth are indicated for injection into the dermis or subdermis for the correction of moderate-to-severe dynamic facial wrinkles and folds, such as nasolabial folds, in adults aged 22 and older.

That sentence matters more than it looks. A dermal filler product can be FDA-approved while a specific injection site is an off-label use. Revive does not describe lip, cheek, chin, jawline or facial balancing treatment with Evolysse as the FDA-approved indication, because it is not. Where a treatment area falls outside the labeled indication, that is discussed during informed consent before anything is injected.

Form vs Smooth

The two products are positioned differently by the manufacturer:

  • Evolysse Form — positioned for lift and support.
  • Evolysse Smooth — positioned for a softer, smoother result.

Which one is appropriate for you, if either is, depends on anatomy, tissue characteristics, treatment area and aesthetic goal. Revive selects filler on those grounds rather than on brand preference, and the right answer is sometimes a different product entirely — or a different category of treatment altogether.

What Evolysse Is Not

  • It is not topical. There is nothing to apply, massage in, or add to a routine at home — it is injected by a provider.
  • It is not a serum, a cream or any other form of skincare.
  • The correct name is Evolysse Form or Evolysse Smooth, described as an injectable hyaluronic-acid dermal filler. Hyaluronic acid occurs naturally in the body, but the product itself is a manufactured, cross-linked medical device, and Revive does not market it with "natural" positioning.
  • It is not a biostimulator. Collagen biostimulators such as Sculptra and Radiesse are a different product category that work over months by stimulating a collagen response. Evolysse works by occupying space from the day it is placed.
  • It is not approved for every facial area, and no filler is.
  • It does not reverse or prevent aging. It addresses volume and contour at a point in time.

Safety and What to Expect

All dermal filler injections carry risks. Common effects include swelling, tenderness, bruising, redness, pain and lumps at the treatment site. Rare but serious vascular complications can occur, including tissue injury and vision complications.

Revive reviews warning signs and emergency contact instructions as part of treatment, and every patient receives written aftercare. Contact Revive urgently if you notice worsening or severe pain, skin that turns white, dusky, blue or mottled, or any change in vision.

Longevity varies by product, treatment area, anatomy, metabolism, movement and the amount of product placed. There is no single duration that applies to every filler and every area, so Revive does not publish one.

Where Evolysse Fits at Revive

Product selection is one part of a treatment plan, not the plan itself. If you are considering filler, the useful starting point is the goal rather than the brand:

  • Lip filler — shape, border definition, symmetry and proportion. $650.
  • Facial balancing — full-face planning across cheeks, chin, jawline and more. Plans from $2,250.
  • Dermal fillers — how the filler treatments compare, and when something else is the better tool.
  • PRF EZ Gel — an autologous option often preferred for selected under-eye concerns.
  • Biostimulators — Sculptra and Radiesse, a different category with a different timeline.

Injectable treatments at Revive Beauty Bar are performed by Angelica Alcaraz, BSN, RN, in a physician-directed medical aesthetics practice. Candidacy, product selection and treatment areas are determined at consultation, and consultations are complimentary.
