Not All Dark Patches Are Melasma

These look similar in a mirror and behave very differently under a laser. Working out which one you have is the step that determines everything after it.

Melasma

Symmetrical patches, usually across the cheeks, forehead and upper lip, driven by hormones and sun exposure. It is chronic, it recurs, and it can be worsened by aggressive heat or light. Treated conservatively and always paired with sun protection.

Post-inflammatory hyperpigmentation

The dark marks left where a breakout, an ingrown hair or an injury healed. PIH tends to be more predictable than melasma and often improves within an acne or rejuvenation plan rather than needing a separate one.

Sun-related uneven tone

Diffuse dullness and irregular tone from accumulated sun exposure. Often the easiest of the pigment presentations to improve, and frequently the one patients describe as simply wanting to look brighter.

Discrete sun spots and lentigines

Individual defined spots rather than a diffuse patch. Worth distinguishing from melasma because the treatment approach and the expected pace of change are different.

Melasma Recurs. A Good Plan Expects It.

This page used to promise to fade melasma for good. It cannot, and neither can anyone else's laser. Melasma is chronic and driven in part by hormones and UV exposure, both of which continue after your last session. Sun, pregnancy, hormonal contraception and time can all bring it back.

What a well-run series aims at is a real reduction in how visible the pigment is — and then a plan for holding it. That plan includes daily sun protection and individualized skin care, not because it sounds thorough but because a pigment series without them is working against itself.

Pigment is also the slowest thing on this platform to move. Redness responds faster; active acne responds faster. Judging a melasma series after one session tells you almost nothing.

Why 1064 nm

The Wavelength Is the Whole Argument

Pigment treatment has an obvious tension in it: you are aiming light at melanin in skin that already responds to injury by making more melanin. That is why so many pigment devices narrow their candidacy as skin tone deepens, and why plenty of people in this county have been told a laser is not an option for them.

1064 nm sits at a part of the spectrum where surface melanin absorbs relatively little energy. Combined with the 650-microsecond pulse, that is the physical basis for the Neo platform being designed across Fitzpatrick I–VI rather than a marketing position.

What that does not mean: that it is risk-free, that it beats any particular other device for your presentation, or that candidacy stops being individual. Settings, spacing and whether to treat at all are still assessed for your skin — and in melasma, declining to treat is sometimes the right call.

How a Pigment Plan Runs

    Identify what the pigment actually is

    Melasma, PIH, sun damage and discrete lentigines look similar in a mirror and behave very differently under a laser. This is the step that determines everything after it, and it is why a photo consultation is not a substitute for seeing the skin.

    Assess pigment risk

    Pigment work carries pigment risk. In deeper skin tones, and in melasma specifically, an over-aggressive setting can produce exactly the outcome you came in to fix. Settings are selected conservatively and individualized to your skin type and history.

  3. 03

    Sun protection is part of the plan

    This is not an upsell line. Melasma is driven in part by UV exposure, and a pigment series without daily sun protection is working against itself. We will be specific about what to use and when.

  4. 04

    Your sessions

    Aerolase is generally positioned as low social downtime rather than no downtime. Temporary redness, warmth or other treatment-specific reactions can occur, and what you experience is individual.

  5. 05

    Reassess and maintain

    Pigment moves slowly compared with redness or active lesions. The series is reviewed as it runs, and for melasma in particular a maintenance plan is part of holding the result rather than an admission it did not work.

Can Aerolase remove my melasma permanently?
No. Melasma is a chronic condition that can recur, and no laser cures it — anything promising permanent removal is overselling. What a well-planned series aims for is a meaningful reduction in how visible the pigment is, paired with sun protection and individualized skin care to hold that improvement. Recurrence with sun exposure, hormonal change or pregnancy is normal and is planned for, not treated as failure.
That caution is well founded, and we take it seriously. Melasma can be aggravated by heat and by aggressive light-based treatment. That is exactly why the parameters here are individualized and deliberately conservative, why pigment risk is assessed before treatment rather than after, and why we will decline to treat if we think the risk outweighs the likely benefit for your skin.

Bring it to the consultation. Topical pigment treatments are evidence-based and often work well alongside laser rather than instead of it, but some products affect treatment timing and some should pause around a session. We will coordinate the two rather than ask you to abandon something that is helping.
